Amendment to US passport, Chicago passport agency, Metropolitan Express

AMENDMENT TO US PASSPORT

Effective September 26, 2005, as an additional fraud prevention measure, Passport Services will no longer amend currently valid passports. Instead, customers will be issued replacement passports. If you are in one of the situation, you need to re-apply a new US Passport.

    * You are married and want to change your Last Name.
    * Other information on your passport need to be corrected
    * Any miss printing on your current US Passport

If your US Passport was issued less than one year, please prepare the following documents in your application package:

   1. Re-Application Form DS-5504 (Download DS5504)
   2. The passport to be replaced,
   3. The documentation required for the change (See Instruction on DS5504 Form page 2)

a. Acceptable evidence of name change, such as marriage/divorce/adoption decree; or court order of name change; Or
b. Birth Certificate to prove your correct name, date of birth

   4. 2 Identical new photos.
   5. 2 Letter of Authorization (Please call (312)388-8472 to get the Letter of Authorization)
   6. A money order of $60 payable to Department of State
   7. A money order or a cashier's check of $80 payable to Metropolitan Express (For 2-3 day service)
       Or A money order or a cashier's check of $100 payable to Metropolitan Express (For Next business day service)
   8. A self addressed envelope with postage or account number for return (FedEx, UPS, DHL or EMS are preferred)

SCENARIO

It takes the Department of State 2-3 business days to process. If you use FedEx next day or EMS to apply and return, there will be another 2 days for your mail to go back and forth. Certified mail or priority mail is also acceptable, but it will take longer turn around time.
