Effective September 26, 2005, as an additional fraud prevention measure, Passport Services will no longer amend currently valid passports. Instead, customers will be issued replacement passports. If you are in one of the situation, you need to re-apply a new US Passport.

    * You are married and want to change your Last Name.
    * Other information on your passport need to be corrected
    * Any miss printing on your current US Passport

If your US Passport was issued less than one year, please prepare the following documents in your application package:

   1. Re-Application Form DS-5504 (Download DS5504)
   2. The passport to be replaced,
   3. The documentation required for the change.

       a. Acceptable evidence of name change, such as marriage/divorce/adoption decree; or court order of name change; Or
       b. Birth Certificate to prove your correct name, date of birth

   4. 2 Identical new photos.
   5. 2 Letter of Authorization (Download Here)

   6. A money order of $60 payable to Department of State

   7. A money order or a cashier's check of

     a. $89 payable to Metropolitan Express (For 4-6 workday service)

     b. $139 for 1-2 workday service

     C. $169 for SAME workday servcie.
